Description

The Bridge offers supportive housing and behavioral health services to vulnerable New Yorkers facing behavioral health and substance use challenges. As a recognized leader in developing and operating supportive housing and community-based programs, The Bridge has 70 years of experience helping New Yorkers with mental illness, including those affected by homelessness, incarceration, and institutionalization.

Scope of Position: The Bridge is seeking dedicated and compassionate Case Managers to join our mission of changing lives by offering help, hope, and opportunity to the most vulnerable members of our community. Case Managers provide essential supportive services to formerly homeless adults diagnosed with serious mental illness and/or substance use disorders, helping them maintain stable housing and achieve greater independence. Responsibilities include connecting clients to benefits, healthcare, and community resources; completing timely documentation; conducting home visits; and providing crisis intervention as needed. This role requires strong organizational, communication, and interpersonal skills, as well as a commitment to delivering culturally sensitive, person-centered care in accordance with agency and regulatory standards.

Essential Position Functions:

Provide case management, including benefits assistance, help complete housing applications, provide links to primary and behavioral health care in the community, and housing referrals to adults diagnosed with serious mental illness and/or substance abuse disorders.

Timely completion of required documentation for every client contact as per agency policy, Medicaid, and OMH regulations.

Assist in preparing the program for audits.

Provide crisis intervention, as necessary.

Compliance with requirements, including facilitating appropriate admissions and discharges, maintaining census, documentation, and incident reporting.

Liaise with other departments and outside agencies.

Ability to work with consumers/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences.

Conduct apartment visits in the community on multiple floors, utilizing stairs. Provide escorts to clients throughout the community utilizing public transportation.

Immediately report serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors. Completes incident reports in accordance with Bridge policy. Accounts for consumers, perform diligent searches, and file missing person reports on consumers not accounted for in accordance with policy and procedures.

Attend staff meetings, supervision, and trainings as required.

Any other duties as may be assigned.

Qualifications

Bachelor's Degree preferred.

Experience working with mental health, crisis intervention, and managing trauma-triggered behavior.

Excellent time management and organizational skills.

Team player who is stress-resistant, flexible, creative, and open to feedback.

Willingness to perform CPR/First Aid/AED/ Naloxone.

Preferred knowledge of AWARDS.
