Job description
LifeBridge Health, Inc. seeks a Case Manager RN to coordinate safe, cost-effective care across the continuum. In this role, you will perform comprehensive clinical and psychosocial assessments, develop and implement discharge plans, and collaborate with physicians, nurses, social workers, and community agencies to optimize patient outcomes. The Case Manager RN monitors medical necessity, length of stay, and payer requirements, advocates for patient needs, facilitates access to resources, and supports a compassionate, patient-centered care experience in a mission-driven, innovative environment.
