Job description
UnitedHealth Group in Wisconsin is seeking a Case Manager RN to coordinate patient services through an interdisciplinary process, ensuring appropriate admission, continued hospitalization, and level of care.
You will lead discharge planning from admission, collaborate with the clinical team, patients, and families, and support post-hospital planning while complying with CMS guidelines and maintaining quality and safety.
