n nHow You'll Make an Impact:

nField Based Nurse Case Managers are an important part of the interdisciplinary care team. They are responsible for outreaching and scheduling member face-to-face visits in homes, dialysis clinics, and or physician offices to support higher risk members needing barrier assessments and face-to-face care plan education. In addition, our field nurses develop trusting relationships with the nephrology practices in their markets and work closely with those practices to support improvement in members health outcomes.

nThis is a field-based position meeting with members in their homes and facilities and will be in the field at least 75% of the time. The territory will primarily cover the greater Queens, NY area.

nnnProvide a complete continuum of quality care through close communication with members via in-person or on-phone interaction

n

nnTravel to complete in home and in facility visits with members

n

nnTravel to participate in care planning in facilities & provider offices

n

nnUse nursing assessment skills to identify medical, behavioral, social determinates of health gaps or barriers in treatment plan

n

nnProvide condition education, conduct medication reviews, assist with advance planning and connect to community resources

n

nnDemonstrate ability to establish trusting relationships with members & caregivers and identify innovative strategies to improve and maintain engagement

n

nnParticipate in interactions with interdisciplinary team, treating providers and community resources

n

nnComplete in- patient & post-discharge visits to ensure that discharged members receive the necessary services and resources to focus on reducing readmissions.

n

nnDocumenting and track findings; solid computer and software navigation skills are critical

n

nnComplete and maintain field based safety trainings

n

n nHow You'll Strengthen Our Team:

nQualifications:

nnnCurrent, unrestricted RN license in the state of New York

n

nnAbility to get licensed in other states as needed

n

nn3+ years of RN experience

n

nnRenal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job

n

nnComputer proficiency, to include strong data entry, utilizing MS Office (Word, Excel, PowerPoint and Outlook), and the ability to type and talk at the same time while navigating a Windows environment

n

nnReside in a location that can receive a high speed internet connection or can leverage existing high-speed internet service

n

n nPreferred Qualifications:

nnnBSN or higher level of education

n

nnCertified Case Manager (CCM)

n

nnDiabetic Educator experience

n

nnField-based experience going into homes

n

nnTelephonic case management experience

n

nnExperience working within a managed care environment

n

n nCompensation:

n$85,000 to $97,000 per year
