AMGSpecialtyHospital -Muncie Campus is seeking aFull TimeCase Manager (RN, LPN).TheCase Manager's primary function is to make readily available to the patient,their families and/or significant others, special services in order tofacilitate adjustment of these individual's continuity of care postdischarge. The Case Manager oversees the effective coordination ofadmission and discharges planning, case communications, conferencing, insurancerelations and assurance through managed care systems, and verbal and written reporting.Assures regulations regarding patient rights are fulfilled.Allcandidates must have a Registered Nurse (RN) or Licensed Practical Nurse (LPN) Indiana License in good standing.

Job Requirements
Current Registered Nurse (RN) Indiana License or Licensed Practical Nurse (LPN) Indiana License in good standing.
Current BLS certification.
Current ALS certification.
Minimum of 1-3 years of related work experience as a Case Manager.
Ability to read and communicate effectively in English.
