Healthcare United is seeking a Registered Nurse/Case Manager for a comprehensive home-based health care company committed to providing at-home services in the comfort of the clientâÂÂs home.Hourly rate depends on experience.Locations in Stratford, Hamden, Norwalk, and Middletown.Full-Time/Part-Time - Flexible Hours - Choose 4/40 or 5 days!Per-Diem - Must Case Manage 10 patients and complete at least 5 assessments per month.Qualifications and Skills:40-50 per hourGraduate of an accredited Registered Nurse programCurrent license as a Registered Nurse in ConnecticutAbility to assess and respond to needs of patients and families in varied settings.Ability to make meaningful observations and write comprehensive accurate reports.Benefits:Generous sign-on bonus for full-time 40 hoursMedical, Dental, Vision, Life/Disability, 401k (after 6 months)Ample vacation timeCompany tabletFully stocked nursing bag with suppliesPlease contact Healthcare United directly to learn more about this position.
