Job description
Case Manager – Inpatient Rehabilitation Floor
Schedule: Full Time, Monday - Friday, 8:00‑5:00
Location: Critical Illness Recovery Hospital – Inpatient Post‑ICU Services
Licensure Requirements: Current license in a clinical discipline: Nurse (RN, LPN, or LVN), Respiratory Therapist (RT), or Social Work (licensed per state guidelines).
Preferred Experience: Previous discharge planning or case management in an inpatient setting dealing with critical care or acute care patients.
Key Responsibilities
Develop and implement patient‑specific, timely and safe discharge plans.
Perform verification of utilization criteria, ensuring compliance with facility policies.
Build relationships with payor sources to secure proper reimbursement and promote cost‑attentive care.
Coordinate utilization review compliance with Quality Management to fulfill licensure and accreditation requirements.
Maintain fiscal responsibilities: identify and negotiate reimbursement, review insurance verification forms to minimize risk.
Facilitate multidisciplinary team meetings with physicians, nurses, respiratory therapists, and rehabilitation therapists.
Complete accurate medical record documentation, including utilization review and financial records.
Minimum Requirements
Current licensure as a Nurse, Respiratory Therapist (RN preferred), or Certified Social Work license.
Experience in inpatient case management or discharge planning with critical or acute care patients (ICU, step‑down, med‑surg, ventilated).
Preferred Qualifications
Specific experience in care management and discharge planning.
Working knowledge of insurance industry and government reimbursement.
