Registered Nurse (RN) Case Manager
Location: Savannah, GA | Job Type: Direct Hire or Contract to hire

Pay: $85,000 per year
The Role (Key Responsibilities)
As our Registered Nurse (RN) Case Manager, you will be responsible for making a difference and driving impact. Your daily impact will include: Provides services in accordance with the plan of care.
Makes the initial evaluation visit and regularly reevaluates the patient's needs.•
Initiates the plan of care and necessary revisions.
Prepares clinical and progress notes for each patient visit in a timely manner as per agency policy.
What You Bring (Qualifications) Experience: 2 years of experience in (Field/Industry).
Education: RN License, BSN preferred.
Skills: • Ability to exercise initiative and independent judgment.
Soft Skills: Strong communication, problem-solving, and train other associates
Certifications: BLS
