Job description

Pay: $44/hour (based on experience)
What You’ll Do:
Coordinate patient care plans across the continuum of care
Collaborate with physicians, families, and multidisciplinary teams
Ensure appropriate utilization of resources and timely discharge planning
Advocate for patients to achieve optimal outcomes
What You’ll Bring:
Active RN license (FL)
Experience in case management, discharge planning, or utilization review preferred
Strong communication, organization, and problem-solving skills
