nSUMMARY
nSocial Workers are responsible for the evaluation and treatment of clients admitted for treatment. The Social Worker adheres to specific scope of service for activities related to the evaluation and treatment of clients in the home and community.
nESSENTIAL JOB FUNCTIONS
nTo perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:
nnnEvaluation including gathering data from client records, interviews, observation and dialogue with other team members
n
nnDevelopment and implementation of a treatment plan according to physician orders which includes establishing client goals, continuous re-evaluation of progress and appropriate modification of the plan
n
nnTimely preparation of written documentation of client progress including initial evaluations, progress reports, discharge summaries and other requested reports
n
nnOne to one counseling with client/and or family members in their home or other appropriate setting
n
nnIncreases awareness of client and caregivers about brain injury to better facilitate adjustment and understanding of the process of recovery.
n
nnAttends team conferences and reports on client's progress and current status in therapy as well as collaboratively assisting the team in identifying realistic goals
n
nnCompletes and turns in billing information in a timely manner
n
nnCommunicates effectively with internal case manager
n
nnPerforms other related duties and activities as required.
n
n nSUPERVISORY RESPONSIBILITIES

nMinimum Knowledge and Skills required by the Job
nThe requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:
nEducation
nnnMaster's Degree in Social Work from an accredited school of social work
n
nnMembership in NASW and related clinical experience in a rehabilitation setting with focus on traumatic brain injury and spinal cord injury.
n
n nCertificates, Licenses, and Registrations:
nnnIn MI, certified by the State of Michigan as a social worker
n
nnValid Michigan Driver's license for employees in state of Michigan
n
n nOther Skills and Abilities:
nnnMotor Vehicle in operating condition
n
nnAbility to operate a motor vehicle
n
nnAble to travel distances exceeding 45 minutes
n
n nOther Requirements:
nnTravel as needed
n nPhysical Requirements:
nnMedium Work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
