Job description
Job Title: Registered Nurse (RN) Care Coordinator - Senior Living
Location: Seattle, WA (King, Snohomish & Pierce Counties)
Schedule: Monday-Friday, 8:00 AM-5:00 PM
Duration: 6-12 Months
Program: Senior Living
Job Summary:
Seeking an experienced RN Care Coordinator to support senior living residents through care coordination, clinical documentation, provider support, and interdisciplinary team collaboration. This role involves traveling between multiple senior living facilities during the workday to assist with patient care management, transitions of care, therapy coordination, and quality initiatives.
Requirements: Active RN license (or ability to obtain before start date)
Associate Degree or higher in Nursing
Basic Life Support (BLS) certification
1+ year of clinical experience preferred, ideally in long-term care
Experience with EMR documentation and care coordination
Reliable transportation and ability to travel between facilities
Strong communication and time management skills
Travel: Local travel throughout King, Snohomish, and Pierce Counties during scheduled shifts.
