n nHow you'll make an impact in this role
nnnOrchestrate the overall plan of care from admission to discharge, integrating patient and family preferences into a cohesive strategy across the entire continuum of care.
n
nnServe as the clinical expert in assessing and treating complex needs, including legal issues, neglect, substance abuse, crisis intervention, and psychosocial adjustment.
n
nnPro-actively identify and dismantle barriers to care, advocating for the internal and community resources necessary to ensure patient safety and health equity.
n
nnMaintain a continuous dialogue with the care transition team to reevaluate health plans and provide expert guidance on medical record documentation, complex level-of-care recommendations, and post-discharge options.
n
n nWhat minimum requirements you'll need
nLicensure / Certification / Registration:
nnnClinical Counselor Preferred.
n
nnSocial Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
n
n nEducation:
nnnHigh School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
n
nnAcute care case management experience

nnMSW
