About This Role
The Case Manager Specialty RN - Oncology/Radiology works collaboratively with an assigned panel of physicians to manage patients' specialized needs, focusing on assessment, care management plans, and coordination of health services.

Pay: $65 - $79/hour

What You'll Do
Evaluate and identify members' needs.
Interface with Primary Care Physicians, Specialists, and various disciplines on case management plans.
Monitor and evaluate the effectiveness of case management plans and modify as necessary.
Coordinate the interdisciplinary approach to providing continuity of care.
Act as a clinical liaison with outside agencies and providers.
Prepare reports and communicate program changes to appropriate staff.
Act as a patient advocate and educator for self-care management.
Develop individualized patient/family education plans focused on self-management.
Facilitate patients' return to normal daily activities through education and referrals.
Participate in data collection and analysis of clinical outcomes and customer satisfaction.
