nThe Center for Veterans Issues (CVI) is seeking a compassionate and dedicated Case Manager to join our Suicide Prevention Program. In this rewarding role, you will work directly with Veterans and their families, providing critical support, crisis intervention, care coordination, and advocacy to individuals at risk of suicide.
nThis position offers the opportunity to make a lasting impact by helping Veterans navigate challenges, access resources, and build pathways toward recovery, wellness, and stability.
nWhat You'll Do
nCrisis Intervention & Safety Planning
nnnConduct suicide risk assessments using the Columbia Suicide Severity Rating Scale (C-SSRS)
n
nnDevelop individualized safety plans and Wellness Recovery Action Plans (WRAPs)
n
nnProvide crisis intervention and de-escalation support
n
nnDeliver suicide prevention education and gatekeeper training to Veterans, families, and community members
n
n nCase Management & Care Coordination
nnnConduct comprehensive assessments to identify participant needs, strengths, and barriers
n
nnDevelop and monitor individualized service plans focused on recovery and wellness
n
nnConnect participants with VA and community resources, including:
n
nnBehavioral health services
n
nnHousing assistance
n
nnEmployment resources
n
nnBenefits and supportive services
n
nnAssist Veterans with scheduling appointments and navigating systems of care
n
nnAdvocate for participant rights and access to services
n
n nEducation & Coaching
nProvide one-on-one and group education related to:
nnnSuicide prevention and awareness
n
nnRecovery and resilience skills
n
nnCommunity resource utilization
n
nnBudgeting and financial literacy
n
nnDaily living and coping strategies
n
nnUnderstanding participant rights
n
n nCommunity Outreach
nnnBuild and maintain relationships with VA partners, Veteran service organizations, and community agencies
n
nnParticipate in community events and outreach efforts
n
nnExpand referral networks to ensure Veterans receive comprehensive support
n
n nDocumentation & Compliance
nnnMaintain accurate and timely case documentation
n
nnEnsure compliance with agency policies and federal regulations
n
nnParticipate in team meetings, trainings, and quality improvement initiatives
n
n nWhat We're Looking For
nRequired Qualifications
nnBachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field
n nOR
nnnAssociate degree plus 4-5 years of case management experience involving needs assessment and service planning
n
nnMinimum of two years of case management experience
n
nnValid Wisconsin driver's license with a good driving record
n
nnCurrent vehicle insurance
n
nnProficiency with Microsoft Office applications
n
n nPreferred Qualifications
nnnExperience working with Veterans
n
nnExperience supporting individuals with mental health and/or substance use challenges
n
nnFamiliarity with community resources and services within the region
n
nnKnowledge of ServicePoint or similar case management systems
n
n nSkills for Success

n nWorking Conditions
nnnOffice-based position with approximately 70% local travel
n
nnFrequent visits to participant homes and community locations
n
nnLight lifting (up to 15 pounds) may occasionally be required

nPay Details: $24.00 to $25.00 per hour
