Case Manager - Summit, NJ
Pay Rate: $55-$58/hr
Schedule: Monday-Friday, 8:30 AM-4:30 PM
Location: Summit, NJ
Setting: Acute Care Hospital
Education: BSN required
License: Active RN license required
Certification: CCMC preferred
Job Requirements
Required Qualifications
Inpatient Case Management experience is required.
RN license required.
BSN required.
2 years of acute care experience strongly preferred.
Experience in case management and managed care.
Strong understanding of discharge planning and transitions of care.
Preferred Qualifications CCMC certification.
2+ years of acute-care experience.
Experience with utilization management.
Experience performing quality screenings.
Additional Information
Responsibilities Coordinate patients throughout the acute care hospitalization .
Manage all aspects of the discharge planning and transition process .
Perform utilization management and quality screenings .
Handle inpatient case management responsibilities.
Coordinate care and services to support appropriate patient outcomes and transitions.
