nSalary: $22.00 Hourly

nSummary: Provide case management services to participants in a program serving episodically homeless individuals with relapse histories. Case Management includes initial and ongoing assessment with regular follow up and referrals/linkages to community-based services. Perform home visits & provide group facilitation.

n nPrimary Responsibilities:

nnnEnsure adherence to policies and protocols for programs serving chronically homeless individuals, including guidelines for meals, curfews, meeting attendance, client transportation to appointments, etc.

n

nnDevelop regular program schedule including activities, meetings, meals, etc. Access external resources as appropriate including guest speakers, etc.

n

nnEnsure all participants meet the HUD requirements for entry into the program and that all required documentation is included in the client file.

n

nnConduct initial service needs assessment with all clients entering the program.

n

nnDevelop Individual Service Plan with each participant within three days of entry into the program.

n

nnWork closely with Shelter Staff, Relapse Prevention Managers and Sober Housing staff to identify new program participants.

n

nnMaintain daily contact with clients actively involved in the program.

n

nnEstablish and maintain regular contact schedule with all participants for a period of at least 24 months.

n

nnMaintain appropriate files and case notes for each participant.

n

nnEnsure accurate statistical data is maintained and reported monthly.

n

nnOutreach to treatment programs and other community-based resources to establish contacts for client referral; keep detailed log of all referrals made.

n

nnProvide crisis intervention (24-hour cell phone coverage).

n

nnEngage all clients by understanding and addressing their needs whether within or outside the scope of work.

n

nnAttend & participate in engagement team meetings as requested and communicate effectively with clients and staff in other areas.

n

nnMaintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.

n

nnEnsure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.

n

nnOther duties as assigned.

n

n nKnowledge and Skill Requirements:

nnnBachelor’s Degree in related field. May be substituted with a minimum of 5 years working with the target population.

n

nnSubstance abuse experience required.

n

nnExcellent interpersonal and organizational skills.

n

nnPrevious experience with group facilitation preferred.

n

n nOrganizational Relationship:

nnnDirectly reports to Program Manager.

n

nnDirect reports of this position are none.

n

n nPhysical Requirement:

nnnAbility to drive a vehicle.

n

nnAbility to ascend and descend stairs.

n

n nWorking Conditions: Travel within the state will be required. As part of the responsibilities of this position, the Case Manger will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.

nHybrid Work Option: Hybrid work is permissible in some positions at SMOC depending on the key functions and responsibilities. The Case Manager position is eligible to work from home 0-20% of the week in scheduling coordination with the department manager.

nMonday through Friday: 8:00AM - 4:00PM. Includes a 1-hour unpaid lunch break.

n35 Hours per week.
