Rate: $24 USD per hour

nLOCATION: Owens Campus

nSTATUS: Full Time/ Non-Exempt

nMISSION STATEMENT:

nQUALIFICATIONS:

nnnBachelor’s degree in a Human Services related discipline. Minimum of four years of related employment experience may be substituted for degree requirements.

n

nnExperience working with homeless and/or at-risk veterans preferred

n

nnStrong oral and written communication skills.

n

nnProficiency in Microsoft Office Software (Word, Excel, etc.)

n

nnAbility to communicate and work effectively with a diverse group of residents, staff, and community partners.

n

nnMust have a valid Driver's License, clean driving record, and own registered and insured vehicle for client visits. Mileage reimbursement will be provided and a company vehicle will be available for any transporting of clients

n

nnVeterans' status is preferred

n

n nRESPONSIBILITIES:

nnnProvide direct case management services to veterans within the VA Contract program

n

nnScreen veteran's eligibility for the program and facilitate the intake process

n

nnCoordinate care with VA personnel and others on behalf of participants for eligibility documentation and mental and physical health services.

n

nnLearn and maintain knowledge of community employment agencies and procedures, along with modifications to those procedures.

n

nnAssist veterans in identifying education and training goals and develop plans to meet those goals.

n

nnAssist veterans in identifying mainstream needs and facilitate the process with appropriate service providers to meet those needs.

n

nnAssist veterans to alleviate employment barriers.

n

nnMaintain, record, and report progression of veterans and their families through program participation.

n

nnPerform interventions and crisis-oriented counseling.

n

nnConduct outreach activities, including visits to facilities such as shelters, treatment programs, soup kitchens and other locations where homeless and at-risk families may congregate.

n

nnConduct home visits to provide case management services.

n

nnMaintain client databases and referrals for services and generate reports

n

nnComplete accurate and timely data entry into the HMIS system

n

nnAttend all scheduled team meetings and trainings.

n

nnOther duties and special projects as assigned.

n

n nThis position is subject to grant funding; however, if the funding ends the position may be eliminated. This position has access to confidential and private information. It is essential that confidentiality and privacy be observed.

nPHYSICAL REQUIREMENTS:

nWhile performing the duties of this job, the employee is regularly required to walk, stand, climb or balance, stoop, kneel, crouch, crawl, and reach with hands and arms continuously. The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 75 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust focus. Must have the ability to operate a telephone, a desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
