Case Manager
We are seeking a compassionate and resourceful Case Manager for a rewarding career opportunity supporting mothers and children in a residential setting. This full-time position plays a critical role in helping residents access community resources, develop stability, and achieve their personal goals through ongoing support and advocacy.

General Responsibilities of a Case Manager Provide ongoing case management services and individualized support to residents.
Connect residents with community resources, benefits, healthcare services, housing assistance, employment resources, and other supportive programs.
Assist residents in developing and achieving personal goals through advocacy, coaching, and resource coordination.
Maintain accurate case notes, documentation, and resident records.
Conduct resident assessments and monitor progress toward established goals.
Collaborate with internal staff and external service providers to ensure comprehensive support services.
Occasionally transport residents to appointments, meetings, and community resources as needed.
Ideal Background for a Case Manager Bilingual Spanish/English required
Experience providing nonprofit, education/childcare, advocacy, social services support preferred.
Excellent communication, organization, and interpersonal skills.
Ability to work independently while collaborating effectively with a team.
Flexibility and adaptability in a fast-paced environment.
Proficiency with Microsoft Outlook, Word, and basic Microsoft Office applications.
Valid driver's license and ability to transport residents as needed preferred.
