nCASE MANAGER

nWe are excited to invite a passionate and dedicated Case Manager to join our team! Homes for the Homeless (HFH) is searching for a Case Manager for our Families w/ Children facility in the Williamsbridge area of the Bronx. As the Case Manager, you will play a pivotal role in our organization, assisting families in providing them with the necessary care and resources needed. nABOUT HOMES FOR THE HOMELESS:

nHomes for the Homeless operates shelters for families experiencing homelessness in NYC. Our family residences are more than just a place to sleep. Families can make the most of their stay through accessing an array of onsite programs. Many of these programs are also available to people living in the local community. Our goal is to have a positive impact so families can thrive long after they leave our shelters.

nESSENTIAL DUTIES AND RESPONSIBILITIES:

nnnMaintains and updates digital and physical records of all families as required by governing agencies.

n

nnCollaborate with clients to develop service plans based on an assessment of each family’s needs.

n

nnAssist families with referrals to outside agencies for employment, education, parenting, legal, medical, and mental health services.

n

nnMaintain and update case files bi-weekly to document progress.

n

nnAssist families with moving into permanent housing.

n

nnEnsure that clients maintain an open public assistance case.

n

nnAdhere to all city and state guidelines for maintaining documents.

n

nnConducts unit inspections.

n

nnPerforms other duties as assigned.

n

n nQUALIFICATIONS:

nnnAssociate or Bachelor's degree.

n

nnFamiliarity with case management is a plus.

n

nnKnowledge of Outlook, Word, and Excel.

n

nnBilingual in Spanish or other language is a plus.

n
