nnSchedule: Full-Time | Monday–Friday | Rotating Weekends

nnPay: $45–$53/hour

n nMake an Impact Beyond the Bedside
nAs a Case Manager , you’ll serve as a critical connection between patients, families, providers, payors, and the interdisciplinary care team. You’ll help ensure patients receive the right care at the right time while supporting a smooth transition from admission through discharge.
nWhat You’ll Do
n• Coordinate patient care and collaborate with an interdisciplinary team
n• Develop and monitor individualized treatment and discharge plans
n• Meet with patients and families to establish goals and communication plans
n• Facilitate weekly team conferences and family meetings
n• Coordinate referrals and post-discharge services
n• Monitor patient progress, quality, utilization, and payor requirements
n• Support utilization review and identify opportunities to improve outcomes
n• Help prevent discharge delays by coordinating resources and services
n• Ensure accurate and timely case management and discharge documentation
n• Educate patients and families throughout the rehabilitation process
n• Participate in acute-care transfer reviews and help identify trends
n• Stay current on applicable regulations, standards, policies, and payor requirements
nWhat You Bring
nnnActive license/certification appropriate to your discipline (RN, SW, OT, PT, ST, Rehabilitation Counseling, etc.)
n
nnAbility to independently complete assessments within your scope of practice
n
nnRN candidates: Minimum Associate Degree in Nursing; BSN preferred
n
nnOther eligible healthcare professionals: Minimum Bachelor's degree; graduate degree preferred
n
nnEligible for CCM® or ACM™ certification upon hire or able to obtain certification within 2 years
n
nn2+ years of rehabilitation experience preferred
n

nPay Details: $45.00 to $53.00 per hour
