nThe Front Door, Troy, NY, US
nSalary Range: $18.88 To $19.93 Hourly
nJ oin the Unity House of Troy YAMICA Program as a Case Manager and support young adults throughout Rensselaer County in achieving greater independence, stability, and personal success. This position offers the opportunity to build meaningful relationships while helping individuals access resources, strengthen daily living skills, and work toward educational and employment goals in a supportive, community-based environment.

nResponsibilities
nnnEngage in ongoing, consistent interactions with clients to build trust and support progress toward individualized goals.
n
nnProvide or assist with crisis prevention and intervention when needed.
n
nnAssist clients in identifying and pursuing educational and employment opportunities.
n
nnProvide transportation to appointments and community services as needed.
n
nnMake referrals and connect clients with appropriate supportive services and resources.
n
nnAdvocate on behalf of clients with community agencies and service providers.
n
nnConduct home visits and field-based services as needed.
n
nnTeach and reinforce daily living and independent living skills.
n
nnSupport symptom management and overall wellness.
n
nnEncourage healthy social development and community integration.
n
nnPromote an environment centered on inclusion, dignity, and belonging.
n
n nQualifications
nnnBachelor’s or Associate’s degree in health or human services with one year of experience working with diverse populations preferred.
n
nnHigh school diploma or GED with at least two years of relevant experience required.
n
nnValid, clean New York State driver’s license required.
n
nnMust have reliable transportation available for daily work responsibilities.
n
