Job description
SCO Family of Services in New York, NY seeks a Case Planner to work with families to support safety, permanency, and well-being for children and youth. You will develop and implement service plans, assess progress, and address barriers to achieve goals.
Responsibilities include initial and ongoing face-to-face contacts, safety checklists, documentation, referrals, and collaboration with foster parents, birth parents, and community partners to stabilize families and advance permanency goals.
