Job description
A nonprofit services organization in New York City is seeking a new Case Planner (Fieldwork) in Queens to provide foster care case management and field-based support that helps advance safety, permanency, and family reunification.
About the Opportunity:
Shift: Day
Schedule: Monday to Friday
Hours: 9am to 5pm or 10am to 6pm or 11am to 7pm
Setting: Hybrid (office & field)
Responsibilities: Create service plans with the child, biological family, and foster family to address safety, behavioral, educational, psychological, and permanency needs
Conduct regular field visits, including foster homes, schools, courts, hospitals, and other required locations
Schedule and supervise mandated family visits and complete in-person interviews with the child, natural family, and foster family
Document case contacts, progress notes, and other required case records in a timely manner
Participate in supervision, Family Court hearings, Family Team Conferences, and other meetings to support permanency goals
Qualifications: At least 1 year of Case Management experience
Bachelor's degree
Experience in Child Welfare and/or Foster Car
Ability to use public transportation to travel throughout New York City
Ability to work flexible hours, including some evenings as needed to meet client needs
Desired Skills: Bilingual English and Spanish skills
Social services background
Proficiency with Microsoft Office, including Outlook and Excel
Knowledge of Connections
Collaborative, flexible, and coachable approach to team-based work
