Job description
Hope For Youth, Inc. in Nassau County, NY is hiring a Case Planner to support at-risk youth and families through the PINS Diversion Program.
This full-time, hybrid role requires in-home visits, coordinated services, and flexible hours to accommodate families. You will work with youth and families, conducting weekly visits, attending school and court meetings, and delivering life coaching and parent skills training while documenting progress and adhering to program standards.
