Case Planner opportunity available in Brooklyn!
One of our clients, a large, non-profit organization, is seeking Case Planners for their Family Foster Care Division. This position will be starting asap on a long term temporary basis with potential for temp-to-direct hire. The hours are Monday - Friday, 9am - 5pm with potential for over time. This role will require home visits throughout Brooklyn, Manhattan and surrounding areas, as well as court appearances.
Responsibilities will include, but not limited to:
Provide direct case planning services to children in the foster care program
Maintain mandated contacts, develop service plans and connect children and families to appropriate resources
Document progress notes and accurate reports
Make home visits and court appearances, and liaise with ACS and OCFS
Facilitate and actively participate service planning, reviews and family conferences
Qualifications:
Bachelor's Degree
Experience in child welfare, or working with children/juveniles
Computer proficiency in MS Office and ability to quickly learn other database systems
Bilingual Fluency in Spanish or Creole strongly preferred.
