Job Title: Case Worker /Client Navigator II
Location: New York, NY
Duration: 10/26/2026 - 12/19/2026
Shift: 7.00 Hours/Day | 35.00 Hours/Week
Pay Range - $30/hr - $32/hr
SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES:
The Client Navigator is responsible for the maintenance of data, tracking of patients, appointments and health outcomes with the goal of improving patient care and experience.
Assist with patients presenting to Metropolitan Hospital form the point of screening to diagnosis.
Facilitates care by coordinating outside/in-house referrals, and ensuring the necessary information is gathered for complete patient care, including outside medical records, mammograms, and pathology results as the point person to contact
Provides navigator support for newly diagnosed patients, and be present in the outpatient facility when the practice is in session as part of the care team. Evaluates barrier to care.
Refers to the genetic testing program by interviewing patients, determining insurance status, processing financial hardship forms, and communicating with the testing company ensuring a smooth process. Referral to appropriate providers for patients with high risk lesions. Referral to genetics professionals for patients with genetic risk
Maintains patient database of all presenting patients including appointments, work up, stage of diagnosis, and survivorship care plans
Responsible for the maintenance of records, patient data, meeting minutes to maintain compliance
Facilities the financial clearance, and counseling process for pre-operative breast surgery patients.
Monitors the tumor registry to identify breast cancer data and use appropriately for presentation and grant submission as needed.
Maintains effective ongoing communication with clinicians, and all staff to ensure patient and work flow challenges are addressed and mitigated and corrected.
Establishes and maintains relationships with Metropolitan Hospital departments to ensure the most pleasant and optimal patient experience and satisfaction within all points of care, and reports gaps and challenges.
Identifies, organizes, and participates in community outreach activities including participation in educational programs each calendar year with focus on breast disease, breast cancer education, prevention, and/or early detection
Schedules and coordinates meetings with providers and administrators to address breast program concerns.
Verifies insurance information and authorizations via telephone and electronically
Monitors Referral Work Queues to triage and schedule patients when needed
Enrolls patients in MyChart
Reports issues of concern to the Program Leader, Associate Directors, and provides recommendations as needed.
QUALIFICATIONS FOR THE JOB: Bilingual/Spanish
Special Equipment/Machines Operated: Computer
Educational Level: High school diploma or equivalent required; associate or bachelor's degree preferred
Knowledge, Skills, Abilities and other Requirements:
• Bilingual English/Spanish Proficiency with EPIC scheduling tools , including Snapboard and referral work queues
Strong organizational skills and attention to detail
Ability to manage high-volume scheduling in a fast-paced environment
Effective communication and collaboration skills
Ability to prioritize competing demands and meet daily deadlines
Ability to work under pressure
Ability to fill in for any OR scheduler staff when necessary
Years of Experience: Minimum of 2 years of experience in health care OR or procedural scheduling
