Job description
Oak Street Health is seeking a Center Medical Director to lead the clinical team at the Lincoln Crossing Center in Chicago Heights, IL. The CMD will dedicate approximately 80% of time to direct patient care and 20% to administrative duties, guiding providers, and implementing Oak Street’s care model.
The ideal candidate is a MD/DO/APN/PA with board certification in Internal/Family Medicine, 5+ years outpatient leadership, and a passion for improving care for older adults.
