Job description
UofL Health - Frazier Rehab is seeking a Rehab Nurse Navigator to support, coordinate, and educate across the rehab continuum from admission through discharge. The role focuses on improving patient outcomes, coordinating care with physicians, nursing, and therapy teams, and promoting staff education and competency.
The position requires an ADN with 2+ years in rehab nursing; BSN is preferred. Proficiency in EMR systems and Microsoft Office is expected.
