Job description
Houston Methodist Sugar Land Hospital is seeking a Certified Case Manager to plan care transitions, discharge planning, and ensure high-quality patient outcomes on designated units.
The CM will precept staff, collaborate with physicians and payors, and lead efforts to improve workflow, education, and patient satisfaction.
Strong nursing background with five years hospital experience and Texas RN licensure required; magnet-certified or eligible.
