Home Health Aides - Join Our Team at SYNERGY HomeCare of Woodbridge!

As a Certified Home Health Aide (CHHA) or Home Health Aide (HHA), you will play a vital role in delivering compassionate and personalized care to individuals in their homes.
As a Home Health Aide, you will be responsible for providing skilled and non-medical care to our clients, including:
Assisting with daily living activities such as bathing, dressing, and grooming
Managing medications and vital signs
Providing companionship and emotional support
Assisting with light housekeeping and laundry
Encouraging and promoting independence in clients
Responsibilities:
Provide high-quality, patient-centered care to clients in their homes
Develop and maintain positive relationships with clients, families, and caregivers
Collaborate with the interdisciplinary care team to create and implement individualized care plans
Maintain accurate and detailed records of client care and services provided
Participate in ongoing education and training to stay current with industry best practices
