Certified Home Health Aide (HHA)STATUS: Full TimeLOCATION: Rochester Regional Health Home Care - NewarkSCHEDULE: Day/Evening/Nights, ability to create your own schedule. Must work 16 hours a month on the weekends.As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.AttributesCompassionate, patient focused and dependable professionalCritical thinking skills, decisive judgment and the ability to work with minimal supervisionExceptional documentation and communication skillsAbility to read, write and speak English, ability to follow verbal and written instructionsResponsibilitiesPatient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errandsCommunication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularlyDocumentation. Accurately and completely document client care on activity sheetCompliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirementsEducationHHA - Home Health Aide certification - New York State Education Department (NYSED), NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)Physical RequirementsM - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.Any physical requirements reported by a prospective employee and/or employee's physician or delegate will be considered for accommodations.PAY RANGE: $18.65 - $20.00CITY: NewarkPOSTAL CODE: 14513The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
