Care Manager (Medical Assistant)Chronic Care Management (CCM)

Location : Remotely

Employment Type: Full-Time / or start Part time

Schedule : Monday–Friday | Daytime Business Hours or Evenings if preferred

Compensation : $22–35 per hour, based on experience

We are building a new Chronic Care Management (CCM) division dedicated to helping patients living with multiple chronic conditions receive proactive, compassionate, and personalized care between office visits.

Our mission is to improve patients' quality of life, reduce unnecessary hospitalizations, and help patients successfully navigate the healthcare system through education, care coordination, and ongoing support.

Position Summary

As a Care Manager, you will serve as the primary point of contact for a panel of patients with chronic medical conditions.

Unlike a traditional Medical Assistant role, this position focuses on building long-term patient relationships, coordinating care, providing education, and helping patients stay engaged in their healthcare.

Working under the supervision of a Nurse Practitioner, you will collaborate with physicians, specialists, pharmacies, caregivers, home health agencies, and community resources to ensure patients receive coordinated, high-quality care.

This role is ideal for someone who enjoys talking with patients, solving problems, staying organized, and making a genuine difference in people's lives.

Primary Responsibilities: Patient Engagement

Conduct monthly Chronic Care Management (CCM) phone visits

Build trusting, long-term relationships with patients and caregivers

Educate patients regarding chronic conditions, medications, and treatment plans

Encourage patients to actively participate in managing their health

Identify barriers to care and help patients overcome them

Support patients in achieving individualized health goals

Care Coordination

Assist in developing, maintaining, and updating individualized care plans

Coordinate care with primary care providers, specialists, pharmacies, caregivers, home health agencies, and community resources

Assist with medication reconciliation and promote medication adherence

Schedule follow-up appointments and facilitate referrals when appropriate

Support continuity of care following hospitalizations or significant changes in health status

Clinical Support

Monitor patient progress through regular communication

Recognize changes in patient condition and promptly communicate concerns to the supervising Nurse Practitioner

Escalate clinical issues requiring provider evaluation

Support chronic disease management initiatives

Documentation & Compliance

Accurately document all patient interactions in the electronic medical record

Maintain Medicare-compliant CCM documentation

Update patient care plans and health records

Maintain patient confidentiality and HIPAA compliance

Participate in quality improvement initiatives and program development

Required Qualifications

Certified Medical Assistant (CMA) or Medical Assistant with relevant healthcare experience. (RN/LVN is a +)

Minimum of 2 years of clinical experience in primary care, internal medicine, family medicine, geriatrics, home health, or a related healthcare setting preferred

Excellent verbal and written communication skills

Strong organizational and time-management abilities

Comfortable speaking with patients by phone throughout the day

Strong computer skills and experience with electronic medical records

Ability to manage multiple priorities while maintaining attention to detail

Ability to work independently while collaborating with a multidisciplinary healthcare team

Passion for helping patients improve their health and quality of life

Preferred Qualifications

Experience with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Transitional Care Management (TCM), or care coordination

Experience coordinating care across providers and healthcare settings

Familiarity with care management software and electronic medical records

Bilingual candidates are encouraged to apply

Remote or hybrid work, depending on organizational needs

Reports directly to a supervising Nurse Practitioner
