nJob Summary:
nWe are seeking compassionate and reliable Certified Nurse Aides (CNA) to join our Home Health and Hospice team. The CNA will provide high-quality, patient-centered care to clients in their homes or hospice setting under the supervision of a Registered Nurse (RN) or Licensed Vocational Nurse (LVN).
nThe ideal candidate is dependable, caring, and committed to supporting patients with dignity and respect during their care journey.
nKey Responsibilities:
nProvide personal care services including bathing, grooming, dressing, and toileting
nAssist patients with mobility, transfers, and ambulation
nMonitor and report changes in patient condition to supervising nurse
nAssist with vital signs (temperature, pulse, respiration, blood pressure)
nSupport patients with activities of daily living (ADLs)
nMaintain a clean, safe, and comfortable environment for patients
nFollow care plans as directed by the RN/LVN
nDocument care provided accurately and timely
nComply with all agency policies, HIPAA, and safety standards
nEducation & Certification Requirements:
nMust be a Certified Nurse Aide (CNA) with a valid CNA license in the state of Texas (or applicable state)
nHigh School Diploma or GED preferred
nCurrent CPR certification (preferred or required depending on agency policy)
nExperience Requirements:
nPrior experience in Home Health, Hospice, Nursing Home, or Hospital setting preferred
nExperience working with elderly, disabled, or terminally ill patients strongly preferred
nFamiliarity with ADLs and patient care documentation
nSkills & Competencies:
nStrong communication and interpersonal skills
nCompassionate and patient-centered attitude
nAbility to work independently in patient homes
nReliable transportation and valid driver’s license (if required for field visits)
nStrong attention to detail and documentation accuracy
