Certified Nursing Assistant CNA - Home Health Services

nJOB PURPOSE:
nAssist the patient and patient caregiver with personal hygiene needs to increase the level of comfort of the patient.
nKEY RESPONSIBILITIES:
nnnProvides competent nursing care to home health patients according to state and industry and profession.
n
nnDemonstrates knowledge of home care for specific patient care skills
n
nnHelps maintain continuity of care by collaborating with the interdisciplinary team
n
nnObtain vital signs of patients as assigned.
n
nnFollow procedures for handling medical emergencies or other incidents that affect the delivery of service.
n
nnReport patient progress and problems to supervisory personnel.
n
nnObserve and report any changes in condition, including skin integrity.
n
nnProvide comfort care related to turning and positioning.
n
nnMaintain a safe, clean and comfortable environment for the patient.
n
nnAssist with meal preparation and related tasks.
n
nnAssist with patient transfer, ambulation and exercise of patients.
n
nnProvide personal hygiene to patients, including bathing, shaving, oral hygiene, skin care, dressing, toileting, and hair care.
n
nnMaintain accurate and current records of visits.
n
nnCommunicate and document tasks in a timely and accurate manner.
n
nnAbility to be self-directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines.
n

nMINIMUM EDUCATION REQUIRED:
nnCertificate of completion of a state recognized Home Care Aide Training Program and supervised clinical practice.
n nMINIMUM EXPERIENCE REQUIRED:
nnOne (1) years of experience working with adults in a healthcare setting.
n nMINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
nnnCurrent registration in the state Nurse Aide Registry as a Certified Nurse Aide in state of practice.
n
nnCurrent CPR certification.
n
nnValid driver's license and automobile liability insurance.
n
n nADDITIONAL QUALIFICATIONS: (Preferred qualifications)
nnHome Health experience is desirable.
