Responsible for the collection of data related to participant status; performance of basic nursing procedures; ongoing collection and documentation of participant data; assistance with treatments and procedures; and assistance with maintenance of patient care equipment. May require access to storage areas where medications are kept and carry out procedures that require the use of non-prescription medications.Job DescriptionEssential FunctionsAll team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Be the difference.Provides participant care based upon individualized plan of care as directed by the Interdisciplinary Team, including the participants' physical, emotional needs, cultural, spiritual, psychosocial, and educational needs.Documents and reports patient status and condition. Maintains and ensures accurate maintenance of participants' medical records and treats records as confidential.Demonstrates effective communication skills in collaboration with other Interdisciplinary Team members to ensure optimal functioning of the center.Identifies opportunities for process improvement and participates in on-going process improvement activities.Performs other duties as assigned.Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.Minimum RequirementsEducation - Completion of a Nursing Assistant or Patient Care Tech programExperience - One (1) year of experience working with a frail elderly populationIn Lieu OfIn lieu of school verification of a nursing assistant training course, may accept current or former state CN A certification as proof of completed state required training at time of hire.Required Certifications, Registrations, LicensesCNA - NURSING ASSISTANT - NCNAKnowledge, Skills and AbilitiesComputer skillsWork ShiftDay (United States of America)LocationCentennial Way PracticesFacilitySeniorCare PACE - UpstateDepartmentAdministrative and GeneralShare your talent with us!
