Job description
Certified Nursing AssistantsWe are hiring Certified Nursing Assistants to deliver in home personal care to elderly clients according to an established Plan of Care. You will support each client's comfort, safety, and daily needs, and keep accurate documentation for the care team. This is an in person role covering the Milwaukee and Waukesha County area, Wisconsin.Key ResponsibilitiesProvide personal care tasks and related paraprofessional services in line with each client's Plan of CareHelp with hygiene, mobility, transfers, and other daily living activitiesSupport clients living with dementia, Parkinson's, or receiving hospice careMonitor client wellbeing and report changes or concerns promptlyComplete accurate, timely care documentationCommunicate clearly with clients, families, and the care teamTravel between client homes within the service area
