Responsibilities include:Providing direct personal care including bathing, grooming, dressing, oral hygiene, and toileting assistanceAssisting members with transfers, ambulation, positioning, and range of motion supportSupporting mealtime service, hydration, and feeding assistance as neededFollowing individualized care plans and member care cards to ensure safe, personalized careObserving and communicating changes in member condition to nursing leadershipAccurately documenting care and observations in PointClickCare (PCC)Assisting with admissions, discharges, transfers, specimen collection, and basic care procedures as assignedTransporting members to activities and appointmentsMaintaining clean, organized member rooms and care equipmentSupporting a positive, respectful, service-focused environment for members, families, visitors, and fellow team membersWhat We're Looking ForRequired qualifications:Active New York State Certified Nursing Assistant (CNA) certificationHigh school diploma or GEDStrong communication and interpersonal skillsAbility to work effectively as part of a collaborative care teamCommitment to professionalism, confidentiality, and compassionate serviceReliable transportationSuccess in this role:Dependable and punctualCompassionate and member-focusedCalm, attentive, and observantTeam-oriented and collaborativeComfortable in a fast-paced care environmentCommitted to delivering exceptional service with dignity and respectSchedule & ExpectationsThis is a full-time evening shift opportunity (3:00 PM – 11:00 PM).
