Comfortable and knowledgeable with point of service (PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/manage care progress updates in EMRs (Net Health/PCC).Education: educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.Interdisciplinary collaboration: collaborate with other rehab staff, including physical therapists and speech-language pathologists, to provide comprehensive care.Must have state license in New JerseyJoin us at Fallsview, and start a career that's not just about work, but about making a meaningful difference in the lives of others.
