Job description
Role: Charge Registered Nurse (RN) responsible for coordinating and delivering clinical care within a PACE center, supporting elderly participants to live independently in the community.
Key Duties
Oversee daily clinical activities and workflows.
Provide clinical leadership and support to the care team.
Manage participants' health conditions in collaboration with providers.
Administer medications, treatments, immunizations, and wound care.
Conduct assessments and contribute to care planning.
Assist with staff onboarding, training, and inventory management.
Support quality and compliance initiatives, participate in on-call rotations.
Qualifications & Skills
Current RN license, graduate of an accredited nursing program.
Preferred leadership experience as a Charge Nurse.
Experience with elderly or long-term care populations.
CPR certification or ability to obtain.
Strong clinical judgment, communication, and organizational skills.
Participant-focused and compassionate approach.
Additional Requirements
Willingness to be vaccinated against specified diseases.
Meet CMS competency standards.
Flexibility to work beyond normal hours and in varied environments.
