As a Home Health Aide (CHHA), you will play a vital role in delivering exceptional home care services to our patients in South Plainfield, New Jersey.Responsibilities:Provide personalized home health care services to patients, including assistance with ADLs, medication management, and other essential care activitiesDevelop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions and coordinationMonitor patient vital signs, report any changes or concerns to the healthcare team, and implement care plans as directedMaintain accurate and detailed records of patient care, including noting progress, outcomes, and any changes or challengesCollaborate with other healthcare professionals, including nurses, therapists, and social workers, to ensure comprehensive careDemonstrate a commitment to excellence in patient care, following all necessary protocols, policies, and proceduresStay up-to-date with industry developments and best practices to continuously improve patient outcomes and care deliveryAbout the Role:As a Home Health Aide (CHHA) with Synergy HomeCare, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
