Job description
CHHA PositionProvide personal care to patients in their homes, including bathing, dressing, and grooming.Assist with mobility and transfers, ensuring the safety and well-being of patients.Administer medication as prescribed by healthcare professionals.Monitor and record vital signs, such as blood pressure and temperature.Assist with medical documentation and maintain accurate patient records.Provide companionship and emotional support to patients.Follow care plans and instructions provided by healthcare professionals.Communicate effectively with patients, their families, and healthcare team members.Strong interpersonal and communication skills.Ability to follow instructions and work independently.Attention to detail and ability to observe changes in patient condition.Knowledge of proper hygiene practices and infection control measures.Looking for CHHA to take home health and/or hospice visits.Home health rate: $50.00/visit.Hospice rate: $60.00/visit.
