Job description
Perimeter Behavioral of Springfield is seeking a compassionate Discharge Planner to help children, teens, and families move smoothly from inpatient care to aftercare.
You will collaborate with therapists, physicians, and families, coordinate with insurance providers, and ensure continuity of care throughout the discharge process. Monday–Friday, 8:00 a.m.–5:00 p.m., with a supportive, team-focused environment.
