Job description
Calumet County's Children's Long Term Support team is expanding in 2027, adding two new positions to assess eligibility and coordinate supports for children and families. Under supervision, you will determine financial and functional eligibility and develop service plans to maximize a child's independence.
You will manage multiple providers, monitor progress, document records, and collaborate with families and other agencies.
