Job description

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager who can help us fulfill our mission. "Delivering compassionate care to enhance lives and improve communities". We are seeking someone who enjoys working with children, ability solve problems, organized and excellent communication skills. Does this describe you? Job Preview at a Glance:

The Case Manager serves as a central support within the interdisciplinary team, working primarily with children. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.

Pay Rate: $17.50-$18.00 hourly

Responsibilities:

Intake screenings (Completing intake paperwork and referrals)

Completing annual behavioral health assessments, including service planning and support planning

Facilitating CFTs and ARTs

Facilitating groups

Identifying gaps in care for our members

Completing SMI and SED evaluation

Qualifications: Minimum of 22-year of age

Bachelor's degree in the behavioral health field plus one year of relevant experience in behavioral health

A combination of experience and education totaling 2 years is acceptable.

Valid AZ driver's license & motor vehicle clearance.

Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive upon hire.
