Tohono O'odham Nation is seeking a Nurse Specialist (Case Management) to provide outpatient case management and self-management education for patients with chronic conditions. The role supports the primary care team and coordinates care across TONHC facilities including Sells Hospital and health centers.

Responsibilities include education, care coordination, scheduling assistance, and follow-up to reduce hospital readmissions while addressing social determinants of health within the TONHC
