Job description
Austin Regional Clinic seeks a Nurse Navigator to coordinate care for patients with complex health needs and to educate and advocate for their wellbeing. You will develop personalized care plans with primary care providers, engage patients and families, and ensure smooth transitions across care settings.
Responsibilities include reviewing risk stratification, contacting patients, attending visits as needed, and documenting outcomes in Epic while supporting plan adherence and patient engagement.
