Job description
Ponos Management Inc. is seeking a Certified Nurse Practitioner to lead chronic-disease care across hematology, rheumatology, and gastroenterology populations in Virginia. The role combines telehealth with in-home visits within a 30-mile radius of specified VA counties.
The NP will coordinate with physicians, social workers, nurses, and care coordinators to deliver holistic, patient-centered care and ensure quality outcomes across the care continuum.
