Job description
Citizens Memorial Home Health Services is recruiting a Behavioral Health Consultant to join our Care Management team, bringing skill, compassion, and a drive to positively impact every patient and family we serve.
You will collaborate with Rural Health Clinic providers to identify patients with medical conditions that are newly diagnosed, not well controlled, or affected by a possible underlying mental health issue.
