Job description
Job Title: Clinic LPN Department: Substance Abuse Location: Gouverneur Hospital Hours Per Week: Full time Schedule: 10 hour shift
SUMMARY
The licensed practical nurse performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a licensed health care provider.
RESPONSIBILITIES
Collects patient data related to patient and family needs
Performs tasks related to the comprehensive plan of care involving the patient and the family.
Performs and documents direct patient and family centered care according to nursing standards of practice.
Responds to patient conditions to achieve positive patient outcomes.
Performs tasks related to discharge planning to ensure safe and appropriate transition of care.
Supervise unlicensed team members in the delivery of care.
Provides world class customer service to patients and families.
REQUIRED QUALIFICATIONS
Graduate of an accredited practical nursing program
Current, unrestricted New York State licensed practical nurse license.
PREFERRED QUALIFICATIONS
One year of post-licensure practice as a Licensed Practical Nurse preferred.

PAY RANGE
$26.02 - $38.13 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
