Job description
Senior Community Care is seeking a Clinic Nurse to deliver compassionate, coordinated care to PACE seniors. The nurse performs comprehensive assessments, administers medications and treatments, monitors chronic conditions, and supports safe transitions of care. Working within an interdisciplinary team, this role collaborates with physicians, therapists, and social workers to develop person-centered care plans, educate participants and families, and promote independence.
